If a coach passenger calls Amtrak and buys a sleeper ticket, is the cost of the coach ticket subtracted from the cost of the sleeper ticket? If that happens then the passenger is paying for a sleeper ticket for the beginning portion of the trip when the passenger actually rode coach. So I guess that the coach ticket is prorated and the remaining portion is refunded. The refunded amount is then applied to the sleeper ticket. If this is how it happens, how is a location chosen to calculate the proration amount which would also be the starting location for the sleeper ticket?

Depends if the current coach ticket is a saver fare (cannot be upgraded), lowest conventional coach bucket fare (no penalty or bonus, simply add sleeper fare), or a higher coach bucket fare (discount coach portion to lowest conventional fare bucket, less any applicable fees).
