One has got to stop short of the "foul mark" for the switch to avoid such misfortunes. ;)

I have not seen those marked out very clearly in this country. In India they paint a tie white marking the foul point.

To agree with earlier posts, failure of the engineer to heed a red board is so fundamental that it calls into question whether the engineer was heeding other rules of the road such as slow orders. The conductor back in the train can probably guess the speed of the train +/- 33% based on sound and the feel of his feet, assuming he cannot look outside the window -- but that's not precise enough to ensure that the engineer is not running 30 mph through a 25 mph slow order.

When there is loss of confidence in an engineer, he or she must be relieved from duty until confidence is reestablished. Passengers wouldn't want it any other way. Trains move in one dimension only.

As a practical matter, the train crew might have to have urine tests after busting a red board. Depends on rules and labor agreements.
