In high school I was on a school bus that lost it's two back wheels on one side (yes, Dave, Jim & Eric, they had school buses back then). Luckily the inside one fell flat and the bus rested on it. The outside wheel went rolling off somewhere.
That is hilariously terrible. If the tech was like it is today, there is a system (Crossfire is one brand) that pushes air between the two tires keeping them at the proper air pressure, so it's possible when the outside dually popped off it also pulled the air lead off of the inside tire and left the valve stuck open so that it deflated. Funny stuff (assuming nobody was hurt).
